This page contains the information needed to obtain a mining permit from the
Arkansas Department of Environmental Quality. The information is contained in sections
that will guide you to the specific information you need based on what type of mining
permit that you require.
We have provided all of the guidance materials and blank forms now used by the
Surface Mining and Reclamation Division. These documents have been provided in Adobe
Portable Document Format (PDF). The blank forms have been created so that you can
actually type data onto the form and then print the completed form for submission.
The filled in forms can not be saved, so we recommend printing extra copies for
