• Complete the Application for Solid Waste Facility Operator License PDF File / Word File
  
  • Any incomplete application received by the Department will be returned for completion.
  • Make sure application is signed and dated.
  • Be sure to include your social security number and use your full legal name or first and last names with a middle initial.
  • Make sure that you don’t forget to indicate the classification and level of license(s) for which you are applying.
• Attach a check or money order in payment of fees incurred in the initial application process. These fees are nonrefundable. Initial fees are $50.00 per classification of license requested (pursuant to Act 1254 of 1997). See the fee schedule below.
• Mail your completed application with fee payment attached to:

  Solid Waste Management Division
  Arkansas Department of Environmental Quality

Important

• To ensure testing eligibility, applications and all fee payments must be received by the Department prior to the examination date.
• The Department will not bill you for the fees involved. Fee payments must be received with applications in order for requests to be considered administratively complete. An invoice stamped PAID will be mailed to you for your records.
• If paying fees for more than one operator, facilities may write one check.
• Licensing fees should be submitted separately and not mixed with other Department fees.
