New Waste Tire Collection Center Permit Application Requirements

  • TP 6 Waste Tire General Permit Notification  (PDF File) / (Word File)
    (Due 30 days prior to operations with the fee of $25.00 per center)
  • Notification letter(s) to local fire authority identifying the collection center locations

Waste Tire Collection Center Permit Annual Update Requirements

  • TP 8 Waste Tire Collection Center Annual Report  (PDF) / (Word File)
    (This report must be completed and returned with $25.00 per center on or before 2/28 yearly.)

Waste Tire Processing Facility Permit Application Requirements

  • TP 4 Waste Tire Processing Facility Permit Application  (PDF) / (Word File)
  • TP 5 Waste Tire Processing Facility Annual Report (PDF) /(Word File)
     (This report must be completed and returned with a $100.00 fee on or before 2/28 yearly.)

New Waste Tire Transporter  License Application Requirements

  • TP 1 Waste Tire Site Notification (PDF) / (Word File)
     (If you do not have a waste tire site, make a notation to that affect on the form, sign and date the form, and mail to ADEQ.)
  • TP 2 Waste Tire Transporter License Application (PDF) / (Word File)
  • ADEQ Disclosure Statement
  • Submit copies of the current vehicle insurance and driver license for all drivers.
    (Required for all new applicants and existing transporters.)
  • Proof of vehicle insurance, current driver license(s), and a fee of $35.00 per vehicle yearly or $250.00 for a fleet.

Waste Tire Transporter License Renewal Requirements

  • TP3 Waste Tire Transporter Annual Report (PDF) / (Word File)
    (Due on or before 2/28 yearly.)
  • Proof of Vehicle Insurance
  • Current Driver License(s)
  • A fee of $35.00 per vehicle yearly or $250.00 for a fleet

Waste Tire Disposal Facility Annual Report
(Required by Regulation 14 and Due on or before 2/28 yearly.)

Annual Fees

All applicants must provide a working telephone number, correct mailing address, and the physical address of the business.  Fee payments must be submitted pursuant to Regulation Number 14.
(Acceptable forms of payment are checks and money orders; do not send cash by mail.)
