ACTION TAKEN: Site investigation on 10/15/12, met with Jackie Craig of Mt. View Water and Waste Water on site, phone conversation with J.R. Henderson of Mt. View Water and Waste Water on 10/16/12, NPDES Reconnaissance Inspection of POTW, letter to J.R. Henderson dated 10/16/12. An on-site investigation was performed on 10/15/12. Hughes Creek was walked from the permitted treatment plant outfall downstream through four riffle pool sequences. A considerable volume of dark gray colored sludge was observed in the pools and coating the stream bed in the riffles. The sludge was not observed to be present upstream of the plant outfall. The treatment plant was determined to be the obvious source of the material. FURTHER ACTION TAKEN/TO BE TAKEN: A permit compliance letter was sent to the Mt. View Water and Waste Water Superintendent dated 10/23/12. Description Comment: 
The complaint stated that several pools in Hughes Creek were filled with sludge from the Mt. View Treatment Plant. The area is karst topography and the surface flow of the creek disappears underground a short distance downstream of the treatment plant.
Location Comment: 
The site is on Hughes Creek on the west side of Mountain View in Stone County. Turn North off of Hwy 66 on the first road west of the old P.D. Building Supply.
